About Omada

Omada is a virtual care program that empowers you to achieve your health goals. With the Omada program, you get personalized support to help you make progress toward your health goals through human coaching, connected devices, flexible digital tools, and personalized insights.

Whether you want to lose weight, reduce your risk for diabetes or high blood pressure, manage diabetes, lower your blood pressure, or address joint and muscle pain, Omada helps you build healthy habits and manage your health between doctor’s visits.

How does the Omada program work?

Omada is a personalized program that surrounds you with the tools and support you need to reach your health goals, whether that’s losing weight, staying on top of diabetes, lowering blood pressure, or improving overall health.

The program includes:

• A dedicated, human health coach for ongoing one-on-one guidance. Coaches are trained to help participants with their health goals and conditions in ways that work for them.
• Connected devices (scale, blood pressure monitor, and/or glucose meter) automatically synced to a participant’s private account.
• Digital tools and learning experiences that help you explore food, activity, sleep, mindset, and other parts of healthy living. You can use the Omada app to track progress, work toward goals, and build skills that support your health.
• A supportive peer community where you can connect with others who are working toward similar health goals. Depending on your program, you may also have access to condition-specific groups and resources.

Who is eligible for the Omada Program

Individuals and/or their adult family members (18+) covered on a CEBT PPO, EPO or HDHP plan and who meet clinical enrollment criteria for the cardiometabolic programs (prevention, hypertension, and diabetes) are eligible for this program at no additional cost. Individuals and/or dependents ages 13+ who meet the clinical criteria for the Musculoskeletal (MSK) program will be eligible under the PPO and EPO plans at no additional cost. Those enrolled in the HDHP plan will have a small fee applied toward deductible for the MSK program.
