About CEBT Health Centers
CEBT members get exclusive access to free or low-cost primary and preventive care through the CEBT Health & Wellness Centers. Members can access care with board-certified medical providers at any of the six locations in Rifle, Glenwood, Gypsum, Loveland, Greeley and Widefield, or virtually with a telehealth appointment. The health centers offer annual physicals, women’s and men’s health, youth and sports physicals, chronic condition coaching, and acute care, including lab services and onsite dispensing of common medications and vaccines. All medical information is private and protected under HIPAA laws.
Employees, spouses, and dependents ages 2 and older who are on the medical plan are eligible to use the virtual and in-person services provided at any of the CEBT Health & Wellness Centers.
There is no cost to patients for services delivered at the health centers. Members on an HDHP plan pay a $45 fee for in-person, non-preventative visits only. Labs processed outside of the health center and referrals to other providers or testing facilities will be billed to insurance and may include patient payment responsibility.
