Member ID Card Request
Before proceeding to order an ID card, please consider the following:
- You can view ID card information through the CEBT APP, or at www.umr.com
- ID cards are not auto-generated at open enrollment unless one of the following applies: You make a plan change, add/remove dependents, or if any of the demographic information on the ID card has changed. If none of these apply, you should not expect a new ID card.
- If you expect an ID card, please consider a 12-14 day turnaround from when it is issued. You can view the issue date by logging into the UMR portal > ID cards
- If you have dependents aged 16+, additional UMR ID cards will automatically be sent for these individuals.
